Ovarian Hyperstimulation Syndrome (OHSS)

Ovarian Hyperstimulation Syndrome (OHSS) is a side effect of drug-induced ovulation in which a woman’s ovaries become enlarged and produce too many eggs as a result of being overstimulated. Medications (generally FSH) can cause ovarian enlargement. As fluid accumulates in the abdomen, this can cause discomfort and lead to blood chemistry problems. Ovarian hyperstimulation most generally happens after being treated with FSH-containing drugs. The condition begins soon after a treatment and will peak within 7 to 10 days thereafter.

OHSS will most likely happen when estradiol levels become high or an abundance of follicles are present. This condition is most common in women with polycystic ovary syndrome. However, only about 5% of women taking FSH-containing medication develop ovarian hyperstimulation and less than 2% have problems that are serious.

Early symptoms of OHSS include abdominal and pelvic pain, sudden weight gain, nausea and vomiting, dehydration, and reduced urination. Fluid may build up in the lungs and cause breathing problems in more severe cases. Very rarely, an ovary ruptures, blood builds up in the abdomen, fluid imbalances can occur, leading to blood clotting problems, lung clots and stroke.

The way to treat a mild case of OHSS is bed rest and stay away from sex. If you have a moderate to severe case, you should be treated in the hospital with intravenous fluids. It most commonly will go away by itself once you begin menstruating. However, it can take longer if pregnancy occurs.

When undergoing treatment with FSH-containing drugs, you should be watched carefully with ultrasound scans. If too many follicles have matured, the hCG injection should be withheld to prevent OHSS.
